Humana Patients — Prior Authorization Required
If you have Humana insurance, you must obtain a prior authorization from your primary care physician (PCP) before your appointment. Please download the Humana authorization form, have your PCP complete it in full, and return it to us by fax or email before your visit. If this form is not completed prior to your appointment, you may be responsible for the full cost of the visit.

Please verify your insurance coverage before requesting an appointment. Some insurance plans require a PCP referral to be seen by a specialist. If your plan requires a referral and one is not obtained prior to your visit, you may be responsible for the full cost of the visit.
